If you have an EEC voucher contact Bri Lewicki for more information on registration. 

The Club offers licensed programs (EEC Program License #9121648) for both before and after school childcare for children in grades K-8. Each year, our program operates at capacity, with a waiting list. Registration is on a first-come, first-served basis. Register as soon as possible to ensure your child's space!

REQUIRED FORMS & POLICIES

Our childcare programming is licensed by the Department of Early Education and Care (EEC). In addition to completing the registration form, you will need to provide the following in order for your child to attend the program:

• CURRENT immunization records and physical exam for your child attending on file at school

• Individual Health Care Plan & Medication Consent Form, if applicable

• PLEASE CONTACT LUDLOW PUBLIC SCHOOL FOR TRANSPORTATION TO AND FROM THE CLUB
